PLAYERS ARE NOT PERMITTED TO PRACTICE OR PARTICIPATE IN EVALUATIONS UNTIL THE CHILD HEALTH FORM OR OTHER ACCEPTABLE PHYSICIAL FORM IS IN THE RYSI OFFICE. The other acceptable form is the NJ Preparticipation Form.
